I think, for the most part, people look better in real life because they are 3-dimensional, so things that are "flattened out" in photos look fuller, less fake. Lighting also has a drastic effect on how someone looks, and lighting in photos is usually artificial... the fact that you perceive the person in real life with "natural" light can automatically make them look much better.
